1. Prospective Egg Donor Questionnaire

Our Prospective Egg Donor Questionnaire consists of the following sections: 

 

  • Personality and Hobbies
  • Education and Occupation
  • Medical History
  • Fertility History
  • Genetic History
  • Family History

 

Your responses to these questions will be added to your online profile for intended parents to review.

2. FDA Questionnaire

The Food and Drug Administration (FDA) Questionnaire consists of 50 YES/NO questions required by the FDA. The FDA Questionnaire will not be shared with intended parents; this is only for our records.

3. Upload Photos and Video

Lastly, we ask you to upload 10-15 photos of yourself and one quick introduction video. The photos will be added to your online profile and the video will be shared with intended parents upon request.

Are You Ready to Apply?

Becoming an egg donor is a rewarding journey, yet it’s a significant commitment with several considerations. Before diving in, let’s make sure you’re well-informed about the process and your responsibilities as a donor.

Take a moment to review the following expectations and confirm that you’re ready to move forward.

  • I am ready to commit to the process: Intended parents place an incredible amount of trust in their egg donor to comply with instructions and make their eventual pregnancy a success.
  • I will respond in a timely manner: It's important to respond to all communications from Donor Nexus within 24 hours. Failure to do so on 3 attempts may result in disqualification.
  • I meet the application requirements: I am a female between the ages of 19 and 29, a non-smoker and non-drug user with a healthy BMI.
  • I will provide truthful information: Complete honesty and accuracy are essential when filling out the egg donor application. 
  • I am willing to self-inject medications: An egg donor cycle will include approximately 12 days of self-administering injections into my lower abdomen.
  • I am able to attend doctor's appointments: You will be required to keep approximately 6 doctor's appointments between 7:30 a.m. and 9:30 a.m.
  • I am comfortable with the egg retrieval procedure: At the conclusion of the cycle, I will undergo an egg retrieval procedure under sedation to remove eggs from my ovaries.
  • I understand that not all applicants are accepted: Egg donation involves a screening process, and not all candidates are selected.
